Connecting Care Worker - Balranald

The Connecting Care Worker plays a vital role in supporting Aboriginal and Torres Strait Islander people, particularly older people and vulnerable community members, to access culturally safe health, aged care, disability, housing, social, and community support services.
Working closely with clients, families, carers, health professionals, and community organisations, the Connecting Care Worker provides advocacy, care coordination, referral support, health system navigation, and practical assistance to ensure individuals receive the services they need to improve their health, wellbeing, independence, and quality of life.
The role promotes a holistic and culturally responsive approach to care that aligns with the values, traditions, and strengths of Aboriginal and Torres Strait Islander communities.
The priority client target group for this position is Aboriginal people aged over 55 years who are identified as frail and/or vulnerable. Aboriginal people aged over 55 years who are functioning independently are not a priority for access to the services provided by the Connecting Care Worker.
This position is located in Balranald provides outreach services across the Wentworth shires and will engage Coomealla Health Aboriginal Corporation in the course of working with clients in the Wentworth area.
Salary up to $69,076 gross per annum plus super
