Care Coordinator

Resthaven Westbourne Park Residential Services is looking for an experienced and motivated Care Coordinator to join the team in a full-time leadership role, providing maternity leave cover until 30 August 2027.

A little more about the role:

• Working collaboratively with the Senior Manager Residential Services and the Residential Services Managers at our Westbourne Park site to provide quality clinical care and services to all residents.
• Providing clinical oversight across the site, ensuring care standards are consistently upheld and any concerns are identified and addressed promptly
• Working in consultation with residents and their families in the planning, delivery and evaluation of the clinical care provided.
• Delivering professional standards of contemporary best practice nursing and care services at your allocated residential aged care home
• Ensuring effective and collaborative working arrangements with visiting medical practitioners and other allied health professionals
• The supervision of nursing staff and personal care assistants

Essential Criteria:

• At least 3 years' experience as a registered nurse
• Registered Nurse Registration with AHPRA
• Previous supervisory experience as a Registered Nurse
• Outstanding leadership skills and the ability to motivate staff to achieve organisational goals
• A demonstrated ability to communicate effectively with staff and residents
• Ability to logically prioritise varying work demands
• Strong organisational and time management skills
• You have an understanding of the ageing process and a commitment to caring for older people
