Care Manager to support older Tasmanians to remain independent, safe and connected within their communities.

Your New CompanyI am currently working to recruit for an organisation that is a values-driven organisation providing high-quality aged care and disability services across Tasmania. With a strong focus on person-centred care, the organisation is committed to helping participants achieve their goals while maintaining independence, dignity and wellbeing. Working alongside a collaborative team of clinical, operational and support professionals, you will play an important role in delivering services that make a tangible difference in people's lives every day.
Your New RoleAs Care Manager, you will oversee a portfolio of community-based participants, ensuring the delivery of high-quality, person-centred care while maintaining compliance with aged care legislation, quality standards and funding requirements. You will work closely with participants, families, support workers and key stakeholders to develop, implement and review care plans, manage participant budgets and ensure services are aligned with individual goals and needs.
Key responsibilities include:

  • Managing a portfolio of aged care participants
  • Conducting participant assessments and reviews
  • Developing, implementing and reviewing care plans
  • Monitoring participant budgets and funding utilisation
  • Coordinating services in partnership with rostering and service delivery teams
  • Maintaining accurate documentation and case records within CRM systems
  • Identifying and managing risks to participant wellbeing
  • Building strong relationships with participants, families and service providers
  • Ensuring ongoing compliance with aged care standards and organisational requirements
  • Participating in service improvement initiatives and team activities

What You'll Need To SucceedTo be successful in this position, you will have experience within aged care, disability services, support coordination or case management and be committed to delivering exceptional participant outcomes.You will also possess:

  • Experience within aged care, community services or disability services
  • Strong case management or support coordination experience
  • Excellent communication and stakeholder engagement skills
  • Strong administrative and documentation capabilities
  • Ability to work autonomously and manage competing priorities
  • Sound understanding of person-centred care principles
  • Intermediate computer skills across Microsoft Office and CRM platforms
  • Relevant qualification or equivalent experience in Aged Care Case Management
  • Current Tasmanian Driver Licence
  • Current Working with Vulnerable People Check / NDIS Screening
  • National Police Check
  • NDIS Worker Orientation Module
