Are you an experienced Case Manager with a strong background in aged care and Home Care Packages? We are seeking a dedicated professional to support older Australians to remain living independently in their own homes while delivering high-quality, person-centred care.
Your new role
This is an exciting opportunity to join a passionate organisation where you will manage a varied portfolio of clients, coordinate services, oversee care planning, and provide ongoing support to clients and their families.As a Case Manager, you will be responsible for:

  • Conducting comprehensive client assessments and reviews
  • Developing and implementing individualised care plans
  • Managing Home Care Package budgets and monitoring expenditure
  • Assisting with financial data, budget tracking and client statements
  • Coordinating service providers and multidisciplinary care teams
  • Monitoring client outcomes and adjusting supports as required
  • Ensuring compliance with Aged Care Quality Standards and relevant legislation
  • Advocating for clients and supporting them to navigate the aged care system

What you'll need to succeed
To be successful in this role, you will have:

  • Previous experience managing Home Care Packages (essential)
  • A strong background working within the aged care sector (essential)
  • Demonstrated experience in case management
  • Excellent numerical and analytical skills, with the ability to interpret budgets, financial data and client statements
  • Strong communication and stakeholder management skills
  • Experience working within a client-centred care environment
  • High attention to detail and strong organisational skills
  • Current Driving Licence
  • Current Police Check (or willingness to obtain)
  • NDIS Yellow Card
