About the Position

The Residential Care Officer plays a central role in the delivery of Success Care Provider's accommodation and daily living supports. Reporting directly to the Director and supervising a team of two support workers, the position is responsible for assessing participants' needs, planning and implementing individualised support and life-skills programs, monitoring and reporting on participant progress, and connecting participants with the community and external services that complement their supports. The role ensures that care in the organisation's residential and community settings is safe, consistent and aligned with each participant's goals and NDIS plan.

Key Responsibilities

  • Assess participants' needs and plan, develop and implement individualised support, life-skills and educational programs.
  • Interview participants, their families and support networks to assess the nature and extent of their difficulties and support requirements.
  • Monitor, record and report on participants' progress against their goals and support plans.
  • Refer participants to agencies and services that can provide additional health, welfare, housing, employment and training assistance.
  • Assess community needs and available resources for health, welfare, housing, employment, training and other services relevant to participants.
  • Liaise with community groups, welfare agencies, government bodies and private businesses on participant and community matters, and promote awareness of available resources and services.
  • Support families and provide care and assistance to people with disability in group housing and shared accommodation settings.
  • Assist participants to manage social, emotional and financial challenges in their daily lives.
  • Prepare submissions for funding and resources, and reports for government bodies and other agencies.
  • Supervise, guide and roster support workers, ensuring supports are delivered safely and in line with participants' plans.

Requirements

  • Australian citizens and permanent residents are encouraged to apply.
  • Diploma of Community Services or an equivalent qualification.
  • A minimum of one year of relevant experience.
  • Sound knowledge of NDIS processes, support planning, progress-note writing and incident reporting, and the ability to prepare clear reports and funding submissions.
  • Strong communication and interpersonal skills, with the ability to build trusting relationships with participants, families and external agencies.
  • Demonstrated leadership and problem-solving abilities, sound judgement under pressure, and availability to work across a residential support roster as required.
