- $105,000 - $110,000 + Super + Salary Sacrifice

As the Clinical Care Partner, you will provide effective and efficient care management, administrative support, and coordination of services for clients receiving individualised Home Care Packages. Through provision of holistic care management for individually funded clients, the Clinical Care Partner will implement a person-centred consumer directed model to support clients in achieving their goals and enable them to live independently in their own homes.

About you:
- Tertiary qualifications in Nursing, Allied Health or a related clinical field
- Current AHPRA registration
- Strong knowledge of the Aged Care Quality Standards, including Support at Home
- A person-centred clinical leader passionate about helping older people live well
