Description
Puntukurnu Aboriginal Medical Service (PAMS) is seeking a passionate and community-focused Elder Care Support Connector to support older Aboriginal and Torres Strait Islander people in accessing aged care services, advocacy, and culturally safe support
About the Role
Working closely with the Elder Care team, you will:
- Engage with Elders, families and community members to raise awareness of aged care services.
- Support clients to identify their needs and connect with appropriate services.
- Assist clients through the My Aged Care registration and review process.
- Advocate for Elders and provide ongoing support throughout their aged care journey.
- Promote access to appointments and other essential services.
- Build strong community relationships and identify local service gaps and opportunities.
About You
You will have:
- Strong connections and relationships within Aboriginal and Torres Strait Islander communities.
- Excellent communication and interpersonal skills.
- Experience working with older people.
- An understanding of person-centred care and trauma-informed practice.
- The ability to work independently and as part of a team.
- Knowledge of, or willingness to learn about, the aged care sector.
- Experience using client or patient information systems is desirable

- Competitive salary from $73,169.81 to $83,964.04 per annum (depending on skills and experience), plus superannuation
- Salary packaging up to $15,900 per year.
