Concept Care is growing — and we’re building a team of Residential Care Officers who are dependable, genuine, and committed to making a real difference for people living with disability in their homes.

About the role

The Residential Care Officer is responsible for providing skilled residential care, supervision and support to NDIS participants living in Concept Care's Supported Independent Living (SIL) environments. The RCO works closely with Care Coordinators, Support Workers, Clinical and Compliance staff, Behaviour Support Practitioners, allied health professionals and other members of the participant's support network.

Your work may include:

  • supporting personal care, household routines, meals, medication assistance and community activities
  • following individual support, care, health and behaviour support plans
  • developing a detailed understanding of each participant’s abilities, communication needs, routines, risks and goals, and contributing observations to support planning and reviews
  • observing changes in participants’ health, wellbeing, behaviour and support needs, and escalating concerns
  • monitoring how well established support strategies are working and sharing feedback with Care Coordinators, clinical staff and other relevant professionals
  • monitoring progress towards participants’ goals and sharing observations with the wider care team
  • helping participants build and maintain independent living skills and take part in decisions about their daily routines and home
  • completing accurate shift notes, handovers, incident reports and other participant records
  • helping maintain a safe, welcoming home and supporting consistent practices across the residential team
  • identifying and reporting safety, accessibility, equipment or maintenance concerns in the residence
  • helping other support workers understand participant routines and approved support strategies, and contributing to clear shift handovers
  • providing complex or high-intensity supports

You’ll be a great fit if you:

  • a relevant qualification in nursing, disability, community services, individual support, health, welfare or a related field is highly regarded but not essential.
  • have experience supporting people with disability, complex support needs or other vulnerable people in a residential, disability, community care or healthcare setting
  • have experience following individual care or support plans and documenting changes in a person’s needs or progress
  • communicate clearly, work well with others and know when to escalate a concern
  • understand boundaries, privacy and safeguarding
  • show respect to participants, families and colleagues
  • are open to feedback, training and growth

Experience in SIL, positive behaviour support or high-intensity supports is highly valued.

An appropriate driver’s licence is required.

Training, safety and compliance

You’ll receive:

  • comprehensive orientation and ongoing training
  • participant-specific training and competency assessments where required
  • support from our coordination and clinical teams when you need help

All roles require the relevant checks, training and competencies, including NDIS Worker Screening, First Aid and CPR, and a Working with Children Check where required.
