Summary

Our client Ibex Care is seeking to add a Community Worker to its compassionate and community-focused team in Guildford, Western Australia. With over half a decade of experience delivering personalised home care, Ibex Care supports individuals and families to live independently with dignity. The successful applicant will contribute to reliable, professional community support while promoting wellbeing, inclusion and access to appropriate services.

The typical job responsibilities will include, but are not limited to the following:

• Assess clients' individual needs and identify appropriate community support services and resources.

• Develop and implement personalised support plans addressing clients' social, health and welfare needs.

• Interview clients to understand their circumstances, challenges, goals and required levels of assistance.

• Monitor client progress and maintain accurate records of outcomes, concerns and support activities.

• Refer clients to suitable health, welfare, housing, employment and community service providers.

• Liaise with community organisations, government agencies and service providers regarding client support needs.

• Promote awareness of available community resources, programmes and services to clients and families.

• Support clients to participate in community activities that encourage independence, inclusion and social wellbeing.

• Assist individuals and families to access appropriate services, information, advocacy and practical support.

• Identify community needs and available resources relating to health, welfare, housing and employment.

• Provide respectful emotional and practical support while maintaining client dignity, choice and independence.

• Collaborate with care professionals and support staff to coordinate consistent client-centred service delivery.

• Prepare progress notes, reports and other documentation relating to client services and community programmes.

• Maintain professional boundaries, confidentiality and compliance with organisational policies and relevant care standards.

Applicants must meet the following minimum requirements to be considered for the role:

• Relevant Diploma-level qualification in community services, welfare or a closely related field, or equivalent relevant experience.

• Minimum 12 months' relevant experience in a Community Worker or similar welfare support role, including client assessment and support planning.

• Strong communication and interpersonal skills for engaging respectfully with clients, families, community organisations and service providers.

• Ability to monitor client progress, maintain accurate documentation and prepare clear reports concerning support activities and outcomes.

• Sound knowledge of community resources, referral pathways and services relating to health, welfare, housing and employment.

• Demonstrated ability to promote client independence, dignity, inclusion and participation within home and community-based support environments.

• Ability to work collaboratively with multidisciplinary professionals while maintaining confidentiality, professional boundaries and client-centred service standards.

• Strong organisational skills with the ability to manage competing client needs, appointments, referrals and follow-up responsibilities effectively.
