Job Description
Community Worker
The Community Worker (Housing Officer) supports the delivery of Brave Minds' housing services by assisting participants to access and maintain suitable accommodation, promoting independent living and positive wellbeing outcomes. The role works closely with participants, families, housing providers, community organisations, and support services to identify housing needs, coordinate tenancy and property-related matters, facilitate access to community resources, and ensure safe, supportive, and well-maintained living environments in accordance with NDIS requirements and Brave Minds Social Services standards.
Daily tasks include
- Assess participants' housing and support needs and assist in implementing individual housing plans that promote independence, recovery, wellbeing, and sustainable tenancy outcomes.
- Coordinate housing placements, property inspections, tenancy documentation, property maintenance, and accommodation setup to ensure Brave Minds' housing services provide safe, compliant, and supportive living environments.
- Monitor participant wellbeing and housing stability, maintain accurate case notes and reports, and ensure compliance with NDIS requirements, organisational policies, and housing program standards.
- Liaise with participants, families, support coordinators, allied health professionals, real estate agents, housing providers, community organisations, and government agencies to facilitate access to appropriate housing and support services.
- Identify and manage housing, health, safety, and wellbeing risks, respond to incidents and urgent accommodation matters, and contribute to maintaining secure and positive living environments across housing sites.
- Promote community participation and social inclusion by connecting participants with relevant mental health, welfare, housing, and community resources, while contributing to the continuous improvement of Brave Minds' Housing Program and related support services.
Requirements:
- Associate degree, Diploma, or Advanced Diploma in Community Services, Mental Health, Disability, Social Work, Human Services, Welfare Studies, Housing Services, or a related field; At least three years of relevant experience may substitute for the formal qualifications listed above.
- At least 1 year of working experience in a related field;
- Current Australian Driver Licence and access to a reliable motor vehicle.
- Strong understanding of person-centred practice, community services, and NDIS service delivery.
Annual salary: $79,435.20 + superannuation (38 hours of work per week)
