WEmpower Services is currently seeking an experienced Family Support Worker to join their team.
The role may involve working across regional NSW, including Central Coast covering post codes 2250-2267, Port Macquarie 2444, South West Rocks 2431 and Coffs Harbour 2450.
Responsibilities:
- Assess clients' needs and help develop and provide suitable support, education and training programs.
- Provide practical support and care to families, children and people with disability.
- Monitor clients' progress and provide ongoing support based on their individual needs.
- Keep records and prepare reports about clients' progress and support needs.
- Refer clients and families to other health, disability, welfare and community services when they need additional support.
- Work with families, community organisations, government agencies and other service providers to coordinate support for clients.
- Help families understand and access available community and disability support services.
- Provide information and guidance to families to help them manage their day-to-day support needs.
Required:
- At least 3 years work experience in the relevant field;
- Excellent communication and problem-solving skills;
- Great attention to details;
- Ability to work effectively with families and other service providers;
- Patience, empathy and a caring approach.
A salary of $80,000 - $85,000 plus super is on offer to the successful candidate.
