Western District Health Service is offering a unique opportunity to combine aged care and hospital pharmacy practice within an innovative role that is helping redefine medicines management across rural healthcare.
This position is ideal for pharmacists seeking variety, professional growth and the opportunity to work across the full continuum of care while remaining actively involved in residential aged care services. Unlike many ACOP positions embedded within community pharmacy, our model integrates pharmacists directly into the hospital pharmacy department, creating exposure to both hospital and aged care practice.
Our hospital-led ACOP model is already attracting national attention, with outcomes to be presented at Medicines Management 2026 through the abstract “Implementing a hospital-led aged care onsite pharmacist (ACOP) model in rural residential care."

What You'll Be Doing
Aged Care On-site Pharmacist Services
Deliver ACOP services across WDHS residential aged care facilities, supporting residents, families, nursing staff and medical practitioners.
Clinical Pharmacy Services
Provide clinical pharmacy services to hospital inpatients and work collaboratively with prescribers, nursing staff and multidisciplinary teams to optimise patient outcomes.
Medicines Optimisation
Conduct medication reviews, identify medication-related risks and support evidence-based prescribing practices.
Medication Safety & Governance
Contribute to medication safety programs, governance activities, audit processes and quality improvement initiatives across the organisation.
Antimicrobial Stewardship
Support antimicrobial stewardship activities across hospital and aged care settings to promote safe and appropriate antimicrobial use.
Service Development & Innovation
Contribute to innovative service redesign projects and help shape the future direction of medicines management within WDHS.
What You Bring
Essential
- Current AHPRA registration as a Pharmacist
- Credentialed Aged Care On-site Pharmacist (ACOP)
Also Considered
- Pharmacists currently completing ACOP training who expect to obtain credentialing within the next month
- Pharmacists currently working within community pharmacy ACOP programs seeking hospital pharmacy experience
- Pharmacists interested in developing expertise across aged care, hospital pharmacy, governance and medication safety services
Classification will be determined based on qualifications, experience and demonstrated capability.
