This rewarding opportunity supports individuals living with severe, persistent and complex mental health conditions who require coordinated, multidisciplinary support beyond standard primary care. You'll play a vital role in delivering person-centred care that promotes recovery, wellbeing and long-term positive outcomes.
The Role As a Clinical Care Coordinator, you will:

  • Manage a clinical caseload and provide direct mental health support
  • Conduct comprehensive assessments, case formulations and care planning
  • Deliver evidence-based therapeutic interventions
  • Undertake risk assessments and develop risk management plans
  • Coordinate care across clients, families, healthcare providers and community services
  • Facilitate referrals and support transitions between services
  • Develop strong community and stakeholder relationships
  • Contribute to clinical governance, reporting and quality improvement initiatives

About You You are a compassionate and experienced mental health professional who is passionate about recovery-focused care.
You'll bring:

  • Experience supporting people with complex mental health presentations
  • Strong understanding of community mental health services
  • Trauma-informed and recovery-oriented practice experience
  • Excellent communication and relationship-building skills
  • Ability to work effectively within multidisciplinary teams
  • A person-centred, non-judgemental and culturally responsive approach

Essential Requirements

  • Relevant tertiary qualification in:
    • Psychology
    • Social Work
    • Mental Health Nursing
    • Occupational Therapy
    • Or a related mental health discipline
  • Current professional registration or eligibility relevant to your discipline
  • Current Australian driver's licence
  • National Police Check
  • Working With Children Check (or applicable exemption)
  • Full Australian working rights

Ideal Disciplines Applications are encouraged from:

Additional mental health qualifications, accreditation or postgraduate training will be highly regarded.
