About the role:  

This role blends clinical leadership with client relationship management, giving you the opportunity to provide assessments, develop care plans, oversee clinical competencies for Support Workers, and foster meaningful partnerships with clients, families, and stakeholders. 

What you’ll do 

  • Conduct clinical assessments and care reviews to ensure safe, tailored, high-quality service delivery 

  • Develop and review care plans, budgets, and support strategies aligned with client needs and funding requirements 

  • Oversee clinical competencies of Support Workers, providing training and guidance as required 

  • Work closely with the Client Engagement Manager to support a seamless onboarding process and ensure an excellent care experience for new clients. 

  • Collaborate with the Rostering Team, Allied Health, and Case Management teams to deliver coordinated and holistic care. 

  • Build and nurture strong relationships with clients, families, referral partners, and community stakeholders 

  • Represent Lumia Care at expos, community events, and networking meetings to promote our services 

  • Champion client satisfaction by conducting regular check-ins, addressing concerns, and driving service improvements 

What we’re looking for 

  • Current AHPRA registration as a Registered Nurse 

  • Proven RN experience, ideally within aged care, disability, community, or primary care settings 

  • Strong clinical assessment, care planning, and training skills 

  • Ability to oversee and maintain Support Worker clinical competencies 

  • Excellent communication and relationship-building abilities 

  • A genuine passion for supporting clients to live safely, comfortably, and independently
